What is Support Coordination?
Navigating the NDIS and addressing your support needs can be a challenging experience. Our Support Coordinators at Easy Living Care Solutions focus on capacity building to strengthen your ability to exercise choice and control, and coordinate supports independently. Our qualified and experienced support coordinators can assist you to:
- Understand and implement your NDIS plan
- Determine what your goals are and identify services or supports needed to achieve these goals
- Develop your skills & independence
- Support you to make your own decisions
- Prepare for NDIS plan reviews
- Connect you with the relevant mainstream & community service providers.
- Referrals for assessments and assistive technology equipment
- Support you in your interactions with government agencies such as the NDIS, Services Australia etc.
- Monitor NDIS plan budgets
Our Support Coordinators are qualified Social Workers who are level 2 registered and able to assist with complex circumstances and multiple service systems.
Support Coordinator costs are covered by your NDIS plan if you have been approved for Support Coordination. Please note: We bill in line with the NDIS Price Guide.
